Question Number 1:
Please provide statements for all purchase cards, ‘p-cards’, or any other credit or debit cards paid out using public funds, used by or for executive directors for the two years April 2016 – March 2017, and April 2017 – March 2018.

Please ensure information is provided for the two separate years.

I am also requesting receipts for these expenses
 
Answer To Question 1:
The Trust does not provide purchasing cards or any other credit or debit cards to Executive Directors.
 
Question Number 2:
Please also provide details of all spending by executive directors using their own money which is then then claimed back from public funds for the two years April 2016 – March 2017, and April 2017 – March 2018.

Please ensure information is provided for the two separate years.

I am also requesting receipts for these expenses.
 
Answer To Question 2:
Please see attached spreadsheet.

The majority of these claims relate to reimbursement of mileage. Miscellaneous travel primarily includes reimbursement of train and taxi fares. All receipts are in storage. To find each claim, identify the relevant receipt, photocopy them and cross reference these to the attached spreadsheet is estimated to bring the time taken to complete this response to over 18 hours in total.

Please also provide details of all spending on behalf of that individual, by the trust, for the two years April 2016 – March 2017, and April 2017 – March 2018.

Answer To Question 3:
There was no further expenditure relating to Directors other than salaries and pension payments in these years. Details of the Directors' remuneration can be found in the Trust's Annual Reports which are available on our website at the following link:

Question Number 4:
Please state if any expenses submitted by the below executives across the two years were rejected, and in each example give all details, including the name of the executive, the cost of the expense submitted, where it was purchased from, what is was for and the reason it was rejected.
