Please list the number of patients per year placed on an end-of-life pathway (LCP or otherwise) or end-of-life care plan between the years 2011 - 2018.
|Answer To Question 1:|
We launched the Trust’s new end of life care plan titled “The personalised care framework for the last days of life” 1st July 2015. The Liverpool Care Pathway had been withdrawn from use in 2014. This was before I commenced working here so cannot confirm the date.
We currently record the number of patient’s known to the End of Life Clinical Nurse Specialist Team, and of those, the number which have a Personalised Care Framework for the Last days of Life completed. However the End of Life Clinical Nurse Specialists team are not aware of every patient that dies. We also do not currently record how many patients known to the HPCT who die in hospital have PCF documentation completed prior to death.
As the Trust does not audit every inpatient death we do not have records for the total number of patients for whom the personalised care framework was utilised prior to death.
I can confirm that between April 18-dec 18 the End of Life Care team supported 209 patients, of which 147 died in hospital. Of these patients, we have recorded that 127 had their end of life care plans documented by means of the personalised care framework for the last days of life.
In additional to this internal audits are carried out every two years (looking at all the “expected” deaths occurring in a month period”)
The following question/s form part of that audit
• Did the patient die on the Liverpool Care pathway? (2013 audit)
• Was the Personalised Care Framework for the last days of life used? (2016 & 2018 audits)
The results were:
2013: 84% of expected deaths audited had their end of life care plan documented using the Liverpool Care Pathway
2016: 43% of expected deaths audited had their End of life care plan documented using the Personalised care framework for last days of life
2018: 59% of expected deaths audited had their end of life care plan documented using the personalised care framework for last days of life
