Request Summary: Financial penalties imposed for higher than expected 30 day readmission rates (2011 - 2018)

Question Number 1:
Since 2011, NHS Trusts in England are subjected to financial penalties for higher than expected 30-day hospital readmission rates. However, information on trust-specific financial penalties is not currently available in the public domain. I hereby request an annual breakdown of all financial penalties imposed on your Trust, starting with financial year 2011/12 and ranging to financial year 2017/18. Specifically, I am requesting the following items listed for each financial year (2011/12; 2012/13; 2013/14; 2014/15; 2015/16; 2016/17; 2017/18):

Total amount withheld (in £) in a respective financial year (i.e. the amount of the penalty)
 
Answer To Question 1:
No information is available for 2011/12 as this was the first year of implementation and the data is not available in a comparable format. During the year 2011/12, contrary to the national guidelines it was agreed with the Trust's host commissioner that an audit of ‘validated’ readmissions would be undertaken and the percentage of ‘avoidable’ readmissions would be rebated to the commissioner. This agreement avoided the continued re-audit of activity or additional reviews. The percentage rebate has been in effect since that audit was undertaken.
