Does your Trust use pen and paper to build patient records? If so, in which departments?
 
Answer To Question 1:
Most departments have some form of paper patient record including wards, outpatients and maternity
 
Question Number 2:
Does your Trust use analogue tape dictation to record patient encounters? If so, in which departments?

Question Number 3:
Who in the Trust is responsible for digitising paper records?
 
Answer To Question 3:
The responsibility sits within the overall governance of Informatics at the Trust. A range of programme boards lead on various elements of digitisation with identified senior responsible officers. This is linked to the digital strategy. Informatics is headed up by the CIO with three CCIOs providing the clinical engagement/link into the organisation.
