Request Summary: Referrals for MSK physiotherapy services

Request Category: Private Individuals

Question Number 1:
Please provide details of the number of referrals received by the Trust for patients to all MSK physiotherapy services for the following financial years:
i) 2016/17
ii) 2017/18
iii) 2018/2019

Answer To Question 1:
Following the go live of Lorenzo as our new EPR in November 2016 we saw changes in the way some referrals were managed on the system, including the specialties that referrals were entered against and what event starts a new referral.

Prior to go live referrals to Physiotherapy were made directly to the service under the specialty of Physiotherapy. This number reduces post go live because referrals are instead forming part of the ongoing original referral rather than a new referral when the patient has been referred on by a Consultant, for example from Plastic Surgery to Physiotherapy. The majority of referrals are from Plastic Surgery but there are other specialties included in this cohort too.

For this reason, when looking at incoming referral volumes we see a shift at this time in both specialty and volume. The monthly volumes are as follows, with the understanding that the drop from 2016-17 to 2017-18 is due to this change.

2016-17 = 2411
2017-18 = 1940
2018-19 = 1923

Question Number 2:
Please confirm or deny whether the Trust provides Musculoskeletal (MSK) services that include first contact physiotherapists in General Practice

Definitions of terms used in questions 2 and 3:
First contact physiotherapists in General Practice* = When a patient has an appointment with a physiotherapist instead of a GP at a GP practice

Answer To Question 2:
No, this sits with Wiltshire Health & Care.

Question Number 3:
Please confirm or deny whether patients can self-refer to physiotherapy services within the MSK services provided by the Trust

Definitions of terms used in questions 2 and 3:
First contact physiotherapists in General Practice* = When a patient has an appointment with a physiotherapist instead of a GP at a GP practice

Patient self-referral**= Patients can refer themselves to physio without seeing a GP or having a referral from anyone else.

Answer To Question 3:
No

Question Number 4:
The amount of time taken between a referral (regardless of its source) to physiotherapist being made and the patient’s first appointment with the physiotherapist for both routine and urgent cases
Please provide details of the average time taken for the following financial years:
i) 2016/17 Routine: Urgent:
ii) 2017/18 Routine: Urgent:
iii) 2018/2019 Routine: Urgent:
c) If this is not available, please confirm if another entity, and if so which, holds this information for patients in the locality.
d) If this information is not held by you or another health body in your area, please confirm if referral to treatment times for physiotherapist appointments used to be recorded and if so, when did this cease to be measured?

Answer To Question 4:
Taking into account the information provided in question 1, the follow figures show the split of Routine and Urgent referrals and their average wait in days.

Please note, average wait time calculations will be skewed due to the reasons above but we are unable to pinpoint the start of the Physio wait as a result of this.

2016-17
Routine = 1960 referrals with average wait of 48 days
Urgent = 451 referrals with average wait of 42 days

2017-18
Routine = 1598 referrals with average wait of 52 days
Urgent = 342 referrals with average wait of 32 days

2018-19
Routine = 1707 referrals with average wait of 28 days
Urgent = 216 referrals with average wait of 29 days
