The figures published quarterly, as required by the The National Health Service (Quality Accounts) (Amendment) Regulations 2017, and directed by the National Quality Board National Guidance on Learning from Deaths, on the number of deaths in the Trust that were subjected to a case record review and were judged more likely than not to have been due to problems in care, for each quarter that they have been published.

This information is published in the Trust Board papers on our website. However for ease of reference the dashboards are extracted and put in the attached document. Deaths with a greater than 50% probability of being avoidable have a Hogan score of 4-6.FOI response.docx
