Can you provide the data you provided to previous FOI requests by Layla Moran or her office. The data you provided to her related to malnutrition rates in hospitals.
Answer To Question 1:
The Trust did not input into this data.
Question Number 2:
Are there any metrics measuring malnutrition rates in admittances into your hospital for the over 65s?

If the response to question two is a yes, please share (for at least a 12 month period):
a. Description of the metric/s measuring malnutrition rates
b. Description of who the metric information is sent to or who regulated by (ie NHSE)
c. The estimated population the hospital serves (split by age ranges, or a focus on 65yr+)
d. The total number of admissions (65yr+)
e. The number of people admitted with signs of malnutrition (65yr+)
f. Any micro level data on the severity of the malnutrition (is there a scale in use)
g. Any micro level data on the cause of the malnutrition (i.e. mental health, gastrointestinal disease)

Are there processes in place to identify malnutrition in an admitted patient?
Answer To Question 4:
We have our own SFT nutrition screening tool which is on Micro Guide under AHP/Nutrition, to identify patients with malnutrition. Please see attachedSFT Nutrition Screening Tool.docx
Question Number 5:
If yes to the above, please provide an outline of these processes
Answer To Question 5:
If they score highly or do not improve with a moderate score, they are referred to a dietitian.
Question Number 6:
An outline of the processes in place to refer a patient who displays malnutrition to a professional who can assist (once they are discharged from hospital setting)
Answer To Question 6:
If a malnourished patient has been under a dietitian while in hospital and the dietitian feels they need ongoing care in the community, they will organise an outpatient follow up
