Question Number 1: Please provide any documentation your have in support of the information requested. How many stillbirths and neonatal deaths in the period 1 September 2018 – 1 September 2020 including the date of death.

Answer To Question 1: 15 Stillbirths and Neonatal Deaths within the specified timeframe. Dates of death will not be supplied please see refusal notice attached.

Question Number 2: Details of why the decision was made to relocate the DAU inside the hospital itself.

Answer To Question 2: The DAU has always been located within the hospital. It was relocated within the maternity service environment to allow for the build of the Birth Centre and to be closer to the Labour Ward environment.

Question Number 3: When did the relocation of the DAU occur?

Answer To Question 3: The relocation happened in May 2021.
