| Question Number 1: Can you please complete the fields in the attached spreadsheet with what you currently hold? The spreadsheet has two tabs, please fill out the first tab if you have a MES contract. In the second tab please fill out the missing information, please use the drop-down list where possible. Please fill out the second tab even if your equipment is not under a MES contract.
