This request is being made to look at methods to improve the reporting rate for adverse events that occur in healthcare systems with the intention of improving patient safety.

714 incidents per month on average, 01/10/22 -> 30/04/23, have been submitted onto datix.
Question Number 4:
What is your ratio of events associated with harm to events associated with no harm please?

For every 374 harmful incidents in the last 6 months, there were 1767 harmless incidents (ratio 1:4.72).
