Answer To Question 1: There were 19 incidents from 01/04/2022-31/03/2023

Question Number 2: The roles and numbers of staff involved in extravasation management per case in your trust.

Answer To Question 2: We are unable to pull this data as this would require reviewing individual patient notes which would take in excess of 18 hours.

Question Number 3: Whether your trust has an extravasation treatment algorithm/protocol and what it entails?

Answer To Question 4: Yes each extravasation incident requires reporting via DATIX.

Question Number 5: If your trust has compulsory DATIX reporting of extravasation injuries, how many incidents have been reported for the last 5 years (1st January 2018 to 1st January 2023)?
