Question Number 1: What Electronic Patient Records system does the Trust use?

Answer To Question 1: Lorenzo

Question Number 2: Does the Trust have a patient portal that allows patients to see their results, letters and appointments.

Answer To Question 2: Not fully, they will be able to see and change their appointments via NHS APP. We are looking to introduce letters online via Dr Doctor.

Question Number 3: If you have surgical departments, how are the operating notes documented? - hand-written? - using specialised software?

Answer To Question 3: Theatreman and Hand written.

Question Number 4: If you have an urology department: - What software system is used to track the JJ stents (Excel, other 3rd party software) - What system or software is used to monitor patients with prostate cancer on active surveillance.~ - If 3rd party software is used what is the associated cost?

Answer To Question 4: 4a – we don’t use any software for tracking JJ stents 4b – we use our electronic patient record (EPR) - Lorenzo 4c - N/A
