Question Number 1:
I am writing to make a request under the Freedom of Information Act for the Trust's official policy for

a) reporting and b) recording of sexual safety incidents, including sexual abuse, sexual harassment or sexual assault through the National Reporting and Learning System (NRLS) or any other relevant system.
 
Answer To Question 1:
All incidents (clinical and non-clinical) are reported through Datix. We have a risk management policy but not specific to reporting and recording these types of incidents. All incidents go through the relevant processes dependent on their nature

The Trust reports incidents of sexual abuse, harassment or assault to the relevant authorities, and records them on its own internal databases.
