I am making a Freedom of Information request for the following:

The number of Datix incidents logged involving patient misidentification, broken down by category of harm if possible, for each of the five following calendar years: 2019, 2020, 2021, 2022, 2023

If the trust did not or has stopped using Datix during this time period, please would you also provide details of incidents involving patient misidentification reported on any equivalent safety reporting system used by the trust.
