Question Number 1: We are writing to request, under the Freedom of Information Act 2000, blank copies of: • Patient in-take registration forms used by Wiltshire Sexual Health, including the questions your service asks about gender/ gender identity/ assigned gender (or sex) at birth, and • A list of the gender/ sex marker options on electronic patient records (if used by Wiltshire Sexual Health) Our freedom of information request is only related to the forms used in processing patient information – intake forms and electronic patient records; we do not require any data about service users. | |
Answer To Question 1: Please find attached the registration and outcome forms with the gender identity questions. Our EPR asks for current gender and then it asks if this gender is the same or different to that gender assigned at birth.
