Question Number 1: Is it protocol to provide a copy of a requested incident report. or do they not provide the requested report.

Answer To Question 1: The Trust do not have an official protocol around sharing a copy of requested reports, we will gain advice from Information Governance for this. Usually if a report is requested and deemed appropriate to share then we will anonymise the report as required and share.
