Question Number 1: Under the provisions of the Freedom of Information Act 2000, I am writing to request the following information. This information relates to the NHS England Emergency Preparedness, Resilience and Response Framework in general and section 7, “NHS incident response levels”, in particular. I am requesting this information from the Trust rather than NHS England or the Integrated Care Board because I wish to know what information is held by the Trust. For clarity I have enumerated the information I am requesting. If you do not hold some of this information then I ask you to confirm explicitly that you do not hold it. I request: Any record held of the number of days the Trust was at Level 1 incident response level between June 2023 and May 2024 inclusive. | |
Answer To Question 1: For information - The incident response levels are identified in the NHS EPRR Framework but in Trusts we manage incidents via Business Continuity, Critical and Major Incident Level 1: • PAC failure – 27th November 2023 – 6th December 2023 –(10 days) The issue was we could not see historical images until the supplier had re-configured the data base. No patients were impacted. • Power Failure – 14th October 2023 (3hrs) This was a power supply failure by SSE with incoming electric to the hospital, so the backup generators were used to supply power to wards. This lasted for around 3hrs. No services were impacted. • Sterilisation plant – 15th November – 5th December (15 days) The plant failed which sterilises equipment for theatres, so we had to receive support from neighbouring Trusts. We reconfigured patient lists so no impact on patients. • Pressure with number of patients attending ED – 9th April – 11th April 2024 (2 days) We declared a Critical incident are we have large numbers of patients attending the hospital and couldn’t discharge enough. | |

Question Number 3: Any record held of the number of days the Trust was at Level 3 incident response level between June 2023 and May 2024 inclusive. | |
Answer To Question 3: Level 3: • Industrial action by Junior Doctors and Consultants over several months (around 22 days) which the regional team required information that was escalated to the national team. Some outpatient appointments cancelled which were rescheduled. | |
