Question Number 1: I am writing to you under the Freedom of Information Act 2000 to request the following information in relation to how the organisation operates its formulary for wound care products. Please may you provide me with: Does the organisation have an 'open' formulary which can be added to at anytime? Yes/No | |

Question Number 2: If No, and the formulary is 'closed' and reviewed on a regular basis, please could you confirm the last date it was renewed to its current status and when is it next open for review? | |

Question Number 3: Who, within the organisation is the most appropriate person to contact in relation to (a) the formulary process and (b) the provision and oversight of wound care? | |
Answer To Question 3: There is no specific person responsible for these processes: (a) the formulary process - any new dressings that are being considered MUST go through the Procurement Dept and be agreed by the clinicians who will be wanting to use them ie Tissue Viability, Burns etc (b) the provision and oversight of wound care - depends on the wound. Wound care oversight would be shared between the Vascular Service, Diabetic Foot Team and Tissue Viability Team. | |
