To request a Mat B1 Form and to book routine appointments, you can email sft.MATERNITY.ROUTINE.ENQUIRIES@NHS.NET. This email address is strictly for non urgent requests.
We invite you to register your pregnancy with us any time after the sixth week of your pregnancy. If you are looking to book your pregnancy, please click here.
Please note: Where the term ‘women’ is used on this page, this refers to all birthing people.
