Your main point of contact during your pregnancy will be your community midwifery team. This team is usually the same for both your care during pregnancy (antenatal care) and care after birth (postnatal care). All mothers will be provided with a telephone number for contacting a community midwife, should advice be required.
The community midwifery teams provide a range of care to ensure the well-being of the mother and baby such as physical and emotional checks, screening and infant feeding support. Occasionally, if a family lives outside of these areas, you may find that the care is provided by a different trust. If this applies to you then this will be discussed with you on an individual basis.
The below table gives details on how the 6 community midwifery teams cover the local areas. The majority of your appointments will be based at the relevant hub.
Please note that the areas that each team covers overlap, so families may not always be cared for by the team detailed below. Families may also have the choice to be seen at a different hub of their choosing, however this may impact on the continuity of care during their pregnancy journey with us.
